Why your neck cracks when you turn your head and what the noise actually means.
A cracking, clicking, or grinding noise when you turn your head is called crepitus, and it is one of the most common reasons people worry about their neck — yet it is usually harmless. The sound comes from one of three mechanisms: gas bubbles forming in the joint fluid when the neck moves (the same "cavitation" process as knuckle cracking), tendons or ligaments snapping over bony bumps as they slide, or joint surfaces grinding against each other as cartilage thins with age. Understanding which one you are hearing tells you whether it matters.
Neck crepitus is so common that imaging studies show over 85% of people over 60 have wear-and-tear changes (cervical spondylosis) visible on X-ray, and most of them have no pain at all. A 2011 MRI study of knuckle cracking showed the "crack" is a momentary gas bubble forming in the joint fluid — and decades of research, including a doctor who cracked one hand for 60 years, found no link between cracking and arthritis. Cervical spondylosis — the age-related wear of the neck joints and discs — shows up on X-rays in about 50% of people in their 50s and more than 85% of people over 60, yet the majority never develop significant symptoms. See a doctor if the cracking is accompanied by pain that lasts more than a few days, numbness or tingling in the arms or hands, weakness, headaches originating from the neck, or dizziness. Pain-free cracking tends to persist for years without changing — it is a normal feature of the joint, not a phase. Cracking that comes with stiffness usually settles within one to two weeks of posture fixes and gentle movement, because the tightness driving it resolves. Cracking linked to an injury or a flare of neck pain follows the underlying problem: most mechanical neck pain improves within four to six weeks with exercise-based care. The pattern to watch is a sound that changes character — a new grinding with pain, or a click that starts catching — because that signals a new mechanical issue rather than the old harmless one.
Occasional, gentle self-cracking when the neck naturally wants to move is unlikely to cause harm. Habitual, forced cracking — twisting hard until something pops — can irritate joints and is best avoided. If you feel a constant need to crack your neck, that urge usually signals stiffness that exercise and posture fixes address better than cracking.
This fear comes from rare reports linked to forceful neck manipulation by practitioners, not from ordinary neck movement or gentle self-cracking. The risk with everyday movement is extremely low. If you ever experience sudden severe headache, dizziness, or neurological symptoms after any neck movement, seek emergency care — but routine cracking does not warrant this worry.
